Telephone Calls The
quickest and easiest way to get information about your health
care is to call your physician's office. Please state if
your call is an emergency. A message will be taken along with
a number where you may be reached. Your chart will be
retrieved prior to the nurse returning your call. The office
nurse will return calls at various times throughout the day. Please contact your pharmacy for routine prescription refills. The pharmacy will contact your physician if necessary. Please call at least 48 hours before your prescription runs out. Routine
referrals for CHP take 5 business days for CHP to process. Main Telephone Directory (All
